PRIVACY STATEMENT

Welcome to Enhanced Steps Support Services 

As you are one of Enhanced Steps Support services (Staff or Service User), we are required to hold some information about you on our files. We take the handling of Personal Data very seriously and this form explains what information we might hold and what we will do with it. If you need more information before agreeing to provide consent. Please contact your key Worker or the manager.

1.0 What information does Enhanced Steps keep about me?

Your file may contain the following information

  • Personal Details such as name, age and gender identity
  • Contact details including address preferred method of contact
  • Details of other people such as your next of kin and emergency contacts
  • Information that you have shared with us about your history support needs and goals
  • Information received from other agencies relating to your support
  • A record of events and meeting whilst you are in our service
  • Information about your benefits, housing medication
  • Complaints
  • Your Photograp

2.0 How long will Enhanced Steps hold my information?

We store your personal information only for as long as necessary to deliver the service in accordance with legal requirements. Where your information is no longer required, we will dispose of it securely and only store anonymised data. We will review what personal information we hold periodically.

3.0 Where my information is held?

Any information about you will be recorded in our password protected IT (computer) system. Sometimes there may be a need to record information on paper. If this is the case, it will be kept secure in a lockable place e.g a filing cabinet.

4.0Who can see my information?

Enhanced Steps staff will have access to your information only where it is necessary for them to perform their roles e.g your key Worker. All of our staff are trained on privacy and Confidentiality. The information about you will not be shared with third parties except for people who need to know more about you to provide you with a support service and to store you safe eg your social worker or GP. This could include the commissioner or funder of the service.

5.0 Can I see my file?

Yes if you wish to view, amend or delete your file, please ask your key Worker or the service manager. Enhanced Steps will respond to your request within 30days. Your request could be declined if there is a risk to you or to other people.

There may be times when you are unable to provide written consent so your key Worker may ask for you to provide verbal consent, your key Worker will ensure that they log on your file when you gave verbal consent and how you gave verbal consent and any evidence of this e.g a screenshot of phone call.
